At age 14, Qualley left home at to board at the North Carolina School of the Arts where she studied dance. She was offered a prestigious position to study at the North Carolina Dance Theatre company, however, she decided not to pursue this, instead moving into modelling. Qualley's first role was in Gia Coppola's film Palo Alto in 2013, which she appeared in alongside Emma Roberts and James Franco. The same year she was also cast in the HBO series The Leftovers. Qualley's CV only continued to grow after she had two films premiere at Sundance in 2017, The Vanishing of Sidney Hall and Novitiate. Her next film, Death Note, was directed by Adam Wingard. One of her biggest roles to date was Pussycat in Quentin Tarantino's Once Upon a Time in Hollywood alongside Leonardo DiCaprio, Bradd Pitt and Margot Robbie.
In 2020, Qualley starred in the feature My Salinger Year, opposite Sigourney Weaver and directed by Philippe Falardeau, which had its world premiere at the Berlin International Film Festival in February 2020. The following year she starred in the heartbreaking mini-series Maid and, on 2022, her film Stars at Noon, which was produced by A24, was lauded at Cannes Film Festival. Fast forward to 2024, and Qualley is enjoying even more success, with roles in Kinds of Kindness and, most recently, The Substance, which she starred in alongside Demi Moore.

In 2021, Qualley starred in the Netflix miniseries Maid, based on Stephanie Land's memoir. When the role of Qualley's character's mother in had not yet been cast, she took matters into her own hands, making a case for Andie MacDowell, her Golden Globe-minted mother, to play the role alongside her. This was a profound experience for Qualley, who told Harper's Bazaar, “When you walk into a room and your mum is there, that does something to you. Not only do you have permission to touch her like family, but rolling your eyes is a built-in response in the same way that tearing up from a well-timed hug is."

Qualley married music producer Jack Antonoff in August, 2023, in an intimate New Jersey ceremony. As is to be expected of the Chanel ambassador, Qualley wore a custom white Chanel halter wedding dress, complete with matching white custom Chanel Mary Jane flats.
A number of Antonoff's famous co-workers were in attendance, including Taylor Swift, Lana Del Rey, Zoë Kravitz and Channing Tatum. Del Ray even wrote a song about Antonoff and Qualley's relationship in the early part of their dating called Margaret.
